Privacy Policy for Care Indeed App

This Privacy Policy outlines the policies and procedures on the collection, use, and disclosure of your information when you use the Care Indeed App application ("App").

We are committed to protecting your personal privacy and ensuring that all personal data collected through the App is handled in accordance with relevant privacy laws. By using the App, you agree to the collection and use of information in accordance with this policy.  If you do not agree to the terms of this Privacy, please immediately cease all use of the Care Indeed App.  Your continued use of the App. Following the posting of changes to this Privacy Policy shall constitute consent to, and acceptance of, those changes.

We collect several different types of information for various purposes to provide and improve our App. While not intended to serve as an exhaustive list, the types of data we collect include the following:


Personal Data

While using our App, we may ask you to provide us with, or otherwise obtain, certain personally identifiable information that can be used to contact or identify you ("Personal Data"). Personally identifiable information may include, but is not limited to:

Information collected by the App. May include information about your past, present, or future physical or mental health and related care services.  This category of information may be protected under federal law, and will be referred to as “protected health information” or PHI.  You have rights related to your PHI in our possession or control if our services are covered by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).  To that extent we will comply with any PHI disclosure rights, obligations, or processes not specified if required or permitted by law.  We will obtain your written authorization before using or disclosing your PHI for purposes other than those set forth herein or as otherwise permitted or required by law.  You make revoke an authorization in writing at any time.  Upon receipt of written revocation, we will stop the use or disclosure covered by the authorization except to the extent of actions already taken in reliance on the authorization.

Non-Personal Data

In addition to collecting personal data as defined above, we may collect information which is not personally identifiable information about users when they use the App.  


Usage Data

When you access the App with a device, we may collect certain information automatically, including, but not limited to, the type of mobile device you use, your mobile device's unique ID, the IP address of your mobile device, your mobile operating system, the type of mobile Internet browser you use, unique device identifiers and other diagnostic data ("Usage Data").


Location Data

We use and store information about your location if you give us permission to do so ("Location Data"). We use this data to provide features of our Service, and to improve and customize our Service. You can enable or disable location services when you use our App at any time, through your device settings.


Tracking & Cookies Data

We use cookies and similar tracking technologies to track the activity on our App and hold certain information. Cookies are files with a small amount of data which may include an anonymous unique identifier. You can instruct your device to refuse all cookies or to indicate when a cookie is being sent. However, if you do not accept cookies, you may not be able to use some portions of our App.

Care Indeed, and each of its subsidiaries and affiliates, may use data collected by the Care Indeed App for various purposes including, but not limited to, the following:

  • To provide and maintain our App;

  • To allow you to participate in interactive features of our App;
  • To provide customer support;
  • To communicate with you;
  • To gather data for the purposes of improving our  App and the services we provide;
  • To monitor the usage of our App;

Care Indeed App will take all steps reasonably necessary to ensure that your data is treated securely and in accordance with this Privacy Policy and no transfer of your Personal Data will take place to an organization or a country unless there are adequate controls in place including the security of your data and other personal information.

We may disclose your personal information in the following situations:

  • Legal Requirements Care Indeed App may disclose your Personal Data in the good faith belief that such action is necessary to:
  • Comply with a legal obligation including, but not limited to, responding to appropriate requests from any federal, state or local agency as required by law and notifying appropriate agencies or authorities of individuals where necessary to prevent, or in the event of, serious threats to health and safety.

  • Prevent or investigate possible wrongdoing in connection with the App
  • Protect the personal safety of users of the App or the public
  • Protect against legal liability
  • To correspond with and coordinate other actions with applicable insurance carriers.

7. Changes to This Privacy Policy

We may update this Privacy Policy to reflect changes to our information practices. If we make any material changes, we will notify you by email (sent to the e-mail address specified in your account) or by means of a notice on the App before the change becoming effective.  Your continued use of the App. after any such changes have been made constitutes acceptance of the updated Privacy Policy.
