Care Indeed's Pro Bono Transitional Nursing & Therapy Support program places licensed RNs, LVNs, and Physical Therapists directly in the homes of qualifying patients who need short-term clinical oversight after discharge or a change in condition. This limited-availability program is provided at no cost to eligible patients, giving case managers and social workers a trusted, frictionless option for patients who fall through the cracks between acute care and independent recovery.
This program is purpose-built for that 72-hour window, with licensed clinicians who assess, monitor, and intervene before a small change becomes a readmission.
This service is provided on a pro bono basis — no insurance authorization delays, no patient cost concerns, no billing friction.
Referring clinicians receive direct, professional communication throughout.
