We Reach Out
Our care coordination team reaches out (usually within one business day) to learn more about the client's needs, preferences, and living situation.
Care Plan
We build a care plan around the client: their condition, home environment, daily routine, and what matters most to them. Not a template; a plan.
Care Begins
Care begins; and if you'd like, we'll keep you updated as things progress. Referring professionals can opt in to receive status updates directly.
