In most cases, Care Indeed can mobilize a care team within 24 to 48 hours of your request — and sometimes sooner when a case manager or social worker contacts us directly during discharge planning. We strongly encourage families to reach out before discharge, even if the date isn't confirmed, so we can begin the matching process early and reduce the scramble at the end.

The 72-Hour Post-Discharge program is designed for the immediate, highest-risk window right after a hospital stay — it's time-limited, intensive, and focused on preventing readmission during those first critical days. Longer-term transitional care, which we call Hospital to Home, is for clients who need continued support over days or weeks while rebuilding strength, managing medications, or recovering from a more complex event. Some clients use both: starting with intensive post-discharge support and transitioning naturally into extended recovery care.

Yes. For clients whose recovery involves wound care, medication administration, injections, or other skilled nursing tasks, Care Indeed can deploy licensed RNs and LVNs as part of the care team. We'll assess clinical complexity during intake and ensure the right level of licensure and training is in place before care begins.

A one-on-one hospital sitter provides dedicated bedside presence during a hospital stay — not replacing hospital nursing staff, but supplementing them. Sitters monitor for falls, confusion, and safety risks; provide calm, consistent orientation for patients experiencing anxiety or disorientation; and notify staff when something changes. This service is most commonly requested for patients with dementia, post-surgical confusion, fall risk, or families who simply want someone they trust present at all times.

We match based on a combination of factors: the clinical complexity of the case, the physical demands involved (mobility assistance, transfer support), the client's personality and history with care, and the availability of staff whose specific experience fits those criteria. For hospital to home transitions, we prioritize staff who have demonstrated comfort with post-acute recovery scenarios — and for medically complex cases, we bring in licensed clinical staff regardless of cost or logistics.
